5. References to other materials.

In bibliographies, MLA or Chicago style entries are preferred. Be sure to include the complete title, author, publisher, publication date, and the edition number if necessary for a book, or the publication date for a periodical.

5.1 References in the text.

In the text, include the complete title, in italics, and the author's name as given in the title the first time the reference appears in the text. It is not necessary to provide publication data in a footnote.

Note that titles and subtitles are separated by a colon.

On subsequent entries, refer to the first citation using either the title or the author. In almost every instance, use the complete title for each reference, and the author's last name in references after the first.

In general, use the shortest reference that will unambiguously identify the document and will not cause the reader to search back through the document for the original reference.

5.2 References in the index.

Provide an entry for the document by title, and an entry for each author, with the title as a sub-entry. See the makeindex manual page for information on how to format index sub-entries.

Each occurrence of the reference must be indexed.

Include the last name(s) of the author(s) in the index entry of the title, and the author(s) name(s) as they appear in the title in their index entries.
